预后营养指数对卵巢癌患者生存时间的评估意义Effects of prognostic nutritional index on the survival of patiens with ovarian cancer

摘要(Abstract)：
目的:探讨预后营养指数(PNI)对卵巢癌患者预后的评估价值,以及PNI与炎症指标的关系。方法:回顾分析在西南医科大学附属中医医院和西南医科大学附属医院2011年1月至2017年7月经病理诊断为卵巢癌的患者86例。Kaplan-Meier生存曲线评估卵巢癌患者PNI对总体生存时间(OS)的影响,采用spearman相关性分析PNI与中性粒细胞/淋巴细胞比(NLR)、血小板/淋巴细胞比(PLR)和淋巴细胞/单核细胞比率(LMR)的关系。单因素和多因素分析确定影响患者预后生存时间的危险因素。结果:卵巢癌患者中,以PNI中位数43作为临界值。低PNI组的累计总生存率显著低于高PNI组(Kaplan-Meier分析显示:χ2=17.819,P≤0.0001)。低PNI与卵巢癌Ⅲ2=17.819,P≤0.0001)。低PNI与卵巢癌ⅢⅣ期的OS显著相关。低PNI组和高PNI组患者的中位OS分别为18个月和37个月。多因素分析显示,PNI与OS具有独立相关性,PNI与NLR和PLR成反比,与LMR成正比。结论:PNI是影响卵巢癌患者预后的独立因素,低PNI提示预后不良,故PNI有潜能作为有效预测卵巢癌患者预后的新指标。
